Confidentiality Agreement
This Non-Disclosure Agreement is entered into and signed as of today by the Medical Clinic (hereinafter referred to as the “Clinic”) and the Patient (hereinafter referred to as the “Patient”), collectively known as the “Parties.” All Parties state that this Agreement is made on their own behalf and on behalf of all affiliated persons or interests they represent.
For good and valuable consideration, the receipt and sufficiency of which are hereby acknowledged, the parties to this Agreement agree as follows:
Purpose: The Clinic and the Patient agree to exchange confidential information for the provision of medical services and the protection of privacy in accordance with laws and best practices.
Definition of Confidential Information: "Confidential Information" refers to any data related to the Patient's health condition, medical treatments, therapies, personal details, or any information disclosed orally, in writing, or electronically, which the disclosing party has designated or identified as "confidential."
Confidentiality Agreement: The Clinic and the Patient agree not to use confidential information disclosed by the other party for any purpose other than the provision of medical services. Confidential information will not be disclosed to anyone except the Clinic staff directly involved in providing those services.
Information Protection: The parties agree to take all reasonable steps to safeguard and preserve confidential information, preventing unauthorized disclosure or use.
Ownership and Rights: All confidential information remains the exclusive property of the disclosing party, and the recipient has no right to use it beyond the scope of this Agreement.
Return of Materials: All documents, records, or other materials provided by the Patient to the Clinic will be returned upon the Patient's request or upon completion of the medical treatment.
Duration of Agreement: This Agreement remains valid for the duration of the medical services provided and terminates two years from the date of signing or upon the completion of treatment, whichever occurs later.
Disputes: This Agreement is governed by the laws of the Republic of Serbia, and all disputes shall be resolved by the competent court in the Republic of Serbia.
Legal Remedies: In the event of a breach of contractual obligations, the parties agree that the affected party may seek legal protection, including an injunction or other appropriate legal remedies.
Notices: All notifications related to this Agreement shall be sent to the addresses listed below or to addresses subsequently designated in writing.
